App to Travel

Everyone requesting permission to or giving notice of travel MUST fill out a travel form.

Your State Association has specifically stated what is and is not sanctioned travel and if there are circumstances where more information is needed than simple notification. Please review your state requirements.
Sanctioned Travel shall be a category of travel established by State Association rule wherein a team travels with the full support of the State Association and is provided all benefits of membership. Sanctioned Travel may be Travel which under the rules of the State Association may occur with nothing more than notification to the State Association, or it may be Travel for which a team, coach or player has requested approval and approval has been given by a State Association with all membership benefits and has the support and assistance of the State Association.
Allowed Travel shall be all other travel wherein the State Association does not provide benefits of membership or assistance. The State Association shall not interfere with such travel, but it should make known to all its members that US Youth Soccer or the State Association does not sanction such travel. State Associations shall adopt rules related to risk management to require that coaches and/or team officials advise parents when travel is not sanctioned and the ramifications of such travel.

Please Note: Permission to travel is free, if submitted at least 14 days in advance. If less than 14 days before the tournament, there is a $10 fee. The $10 fee must be received before your travel papers can be signed.

• Fill out the entire form below, all fields must be completely and accurately filled out according to examples. Please confirm you entries are accurate before Printing.
• Once fields are completed, make sure your printer is connected and print a copy for your records. You will not receive a form from the state.
• Be sure to click the Submit button after printing. Submit sends a copy to the South Dakota State Soccer Association.

• If less than 14 days to the tournament, please send your $10 check with Team Name to Dawn.

• You must fill out one form for each tournament you attend. For League Games, list each date separately in the Event Date field
• Contact Dawn Swoboda if you have lost your travel papers.
